Our 245D Services
The Minnesota Department of Human Services licenses several basic support services under Minnesota Statutes 245D. These services provide the level of assistance, supervision and care necessary to ensure the health and welfare of the person. They are not directed toward training, treatment or rehabilitation; instead, they help people remain in their homes and communities while promoting independence.
Respite Care (In‑Home & Out‑of‑Home)
Respite care offers short‑term relief to family members or primary caregivers. It may be provided in the person’s home or another community setting. Respite allows caregivers to rest or attend to other commitments while ensuring their loved one’s needs are met.
24‑Hour Emergency Assistance & Personal Emergency Response
Our 24‑hour emergency assistance provides access to immediate help when urgent needs arise. Staff can respond via phone or in person to ensure the person’s health and safety at any time of day.
Night Supervision
Night supervision services supply overnight assistance and monitoring by an awake staff member in the person’s home. This service offers peace of mind for those who need support during sleeping hours.
Homemaker Services
Homemaker services include light housekeeping such as cleaning, laundry and meal preparation. They also provide incidental support with home management or activities of daily living.
Individual Community Living Support (ICLS)
Individual community living support helps adults maintain independence in their own homes. Services may include community mobility, accessing resources, safety awareness, budgeting, meal planning and healthy living skills.
Individualized Home Supports (IHS)
Individualized home supports provide training and assistance across four areas: community participation, health and wellness, household management and adaptive skills. Supports are tailored to each person’s goals and may involve skill‑building, supervision and guidance to promote self‑sufficiency.
Integrated Community Supports (ICS)
Integrated community supports are a housing‑based service for adults age 18 and older who live in provider‑controlled settings. This service includes support and training in community participation, health and wellness, household management and adaptive skills. Staff assist with daily living activities (like cooking, cleaning and medication management), help develop community living skills such as transportation and budgeting, and provide social supports to foster relationships and community involvement. The goal of integrated community supports is to help people live more independently while fully participating in community life.
