Eligibility for 245D Services
The Minnesota Department of Human Services licenses 245D services to support people with disabilities and those aged 65 or older. To access 245D basic services, a person must meet the following criteria:
- Be a Minnesota resident and meet state Medical Assistance (Medicaid) requirements.
- Receive a waiver under the Home and Community‑Based Services program. Eligible waivers for 245D basic services include Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD).
- Be referred by a county or tribal case manager.
- Require assistance, supervision or support to maintain health, safety and independence in the community.
