What Is a 245D License?
A 245D license is a Minnesota certification required for providers who deliver Home and Community‑Based Services (HCBS) to individuals with disabilities or special needs. The “245D” refers to the section of Minnesota statutes that outlines the requirements for this license. It is tied to Medicaid waiver programs that fund long‑term care services and applies to organizations that provide direct support services, including residential services, day programs, supported employment, crisis services, respite care and case management. The key purpose of the 245D license is to ensure that services prioritize the health, safety and rights of the people receiving care and to establish minimum standards for staff training, service delivery, documentation and management.
Quality & Compliance
Obtaining and maintaining a 245D license requires providers to meet rigorous standards. Staff members undergo comprehensive background studies, including fingerprint‑based criminal history checks and maltreatment registry reviews.
