Explore Our ServicesSafa Health Care LLC is a Minnesota‑licensed provider of Home and Community‑Based Services (HCBS) under Minnesota Statutes 245D. Learn More About UsShort‑term in‑home and out‑of‑home respite care offers temporary relief for family or primary caregivers and is one of the core basic support services under 245D.
Our team offers 24‑hour emergency assistance and personal emergency response to ensure safety any time.
Night supervision provides overnight assistance and monitoring by an awake staff in the person’s home.
Homemaker services include light household cleaning and incidental assistance with home management and activities of daily living.
ICLS helps adults maintain independence by building skills in community mobility, accessing resources, safety awareness, budgeting, meal planning and healthy living.
IHS provides training and support across four community living areas: community participation, health and wellness, household management and adaptive skills. Services are tailored to each person’s goals.
Integrated community supports offer housing‑based support for adults in provider‑controlled settings, including training and assistance with daily living activities, community participation, health and wellness, household management and adaptive skills.
245D Basic Services are available to Minnesota residents who are disabled or aged 65 and older and who meet eligibility requirements for certain Medicaid waivers. People must have a waiver for Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) or Developmental Disabilities (DD) and be referred by a case manager.
